Corporation

If your business is a corporation, please be sure to provide the following documents:

• Articles of Incorporation
• Corporate Resolution, Bylaws or, board minutes (or a similar document that defines an individual’s relationship to the corporation and their authority)
• Certificate of Assumed Name or Fictitious Name (if using a DBA)
• Copy of government issued identification for all agents

The following forms will also need to be completed and returned to Partners 1st:

• Business Account Application
• Business Account Questionnaire
• Certificate of Beneficial Ownership
• Corporation Resolution
• Internet Gambling
• Overdraft Protection
