If your business is an limited liability company (LLC,) please be sure to provide the following documents:

• Articles of Organization
• Operating Agreement
• Certificate of Assumed Name or Fictitious Name (if using a DBA)
• Copy of government issued identification for all agents

• Business Account Application
• Business Account Questionnaire
• Certificate of Beneficial Ownership
• Limited Liability Company Resolution
• Internet Gambling
• Overdraft Protection
