Sole Proprietorship

If your business is a sole proprietorship, please be sure to provide the following documents:

• Certificate of Assumed Name or Fictitious Name when owners name is not in the business name (file with County Recorders’ office)
• Copy of government issued identification for all agents

The following forms will also need to be completed and returned to Partners 1st:

• Business Account Application
• Business Account Questionnaire
• Certificate of Beneficial Ownership
• Sole Proprietorship Resolution
• Internet Gambling
• Overdraft Protection
